Getting Ready for Your Visit

What to bring

  • Patient registration forms (these are available on our website & offer the convenience of filling them out prior to your appointment to save you time)

  • Photo ID

  • Insurance information and card

  • Previous medical records

  • A list of your current medications

  • Copayment as required by your insurance company

  • Bring a list of questions that you may want to ask the doctor

  • Pharmacy information

Prescription Refills

Prescription refills may take up to 48 hours to be refilled. Please check with your pharmacy to see if they automatically request them on your behalf.

Appointment Cancellation Policy

A 24 hour notice is required for any appointment cancellation. If you fail to provide a 24 hour cancellation window, you may incur a $25 missed appointment fee.

Insurance

We accept most local and many national insurance plans. However, it is your responsibility to understand whether your insurance provider limits or otherwise dictates the doctors you can see or the services you can receive under your coverage.

If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier for payment. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. If your procedure is not covered by insurance, you will be directly billed for the full amount. Please contact our billing office or call your insurance carrier should you have questions.

Please bring your insurance card to every appointment. You must notify our office of any secondary insurance policies you may have.

We participate with many insurance companies; however, it is your responsibility to check with your insurance company to ensure that we participate.
